The purpose of this form is to identify UMS STAFF/ UMS STAFF DEPENDANT/ UMS STUDENTS who are interested in receiving the booster vaccination.

 

The vaccine given for this booster dose is Pfizer.

 

Please take note that the vaccination schedule is through appointment only, and NO WALK INs are allowed.

 

We will call/WhatsApp you to arrange the appointment date & time, this is to avoid overcrowding and wastage of vaccines.
